Abstract: 

Background and Objectives: FFP (Fresh Frozen Plasma) containing all coagulation factors is used to treat coagulation disorders. In the present study, FFP consumption in Children Medical Center was studied to evaluate transfusion indications leading to blood orders some of which might have been unnecessary bringing about inappropriate blood use or unused blood donations.Materials and Methods: In this retrospective study, 1262 FFP order and reservation requests during one year for being administered to 735 patients were reviewed by census method. The results were analyzed by Chi square and SPSS.Results: Out of 1262 requested FFP units, 952 (75.4%) were transfused, and 90 units (7.13%) though thawed were not. Out of 220 reserved units, only 22 units were transfused.Conclusions: Taking more attention in ordering will prevent blood loss and increase economic saving. The blood bank of Children Medical Center does not embark on thawing FFP before its usage is finally confirmed so that blood loss is somehow avoided.

Abstract: 

Mesenchymal stem cells (MSCs) have unique properties, including high proliferation rates, self-renewal, and multilineage differentiation ability. Their characteristics are affected by increasing age and microenvironment. This research is aimed to determine the proliferation, characteristics and differentiation capacity of adipose tissue-derived (AT)-MSCs at many passages with different media. The cell proliferation capacity was assayed using trypan blue. MSCs characterization (CD90, CD44, CD105, CD73, CD11b, CD19, CD34, CD45, and HLA-DR) was performed by flow cytometry, and cell differentiation was determined by specific stainings. Population doubling time (PDT) of AT-MSCs treated with fresh frozen plasma (FFP) and non-FFP increased in the late passage (P) (P15 FFP was 22. 67 ± 7. 01 days and non-FFP was 19. 65 ± 2. 27 days). Cumulative cell number was significantly different between FFP and non-FFP at P5, 10, 15. AT-MSCs at P4-15 were positive for CD90, CD44, CD105, and CD73, and negative for CD11b, CD19, CD34, CD45, and HLA-DR surface markers. AT-MSCs at P5, 10, 15 had potential toward adipogenic, chondrogenic, and osteogenic differentiation. Therefore, PDT was affected by increased age but no difference was observed in morphology, surface markers and differentiation capacity among passages. Cumulative cell number in FFP was higher in comparison with non-FFP in P5, 10, 15. Our data suggest that FFP may replace FBS for culturing MSCs.

Abstract: 

Background: Aprotinin has been used increasingly to reduce postoperative blood loss in open-heart operations; due to the potentialities for complications and high cost, it would seem reasonable to use aprotinin more selectively in small doses in the prime solution of the pump.Methods: We prospectively studied the effect of preoperative low dose aprotinin [2 million units (230mg)] on blood loss and transfusion requirements in patients undergoing cardiopulmonary bypass. One-hundred and fifty patients were randomly assigned to two groups: prophylactic low dose aprotinin (group 1) and a non-medicated control group (group 2). The two groups were comparable in all demographic and operative variables.Results: Postoperative chest tube drainage was significantly decreased in the aprotinin group compared with that in the control group (372.73 mL in group 1 and 482.2 mL in group 2, p<0.05). No significant difference was seen between the two groups in regard to transfusion requirement (p>0.05). The use of fresh frozen plasma (FFP) was significantly less in group 1 than in group 2 (469.87 mL versus 680.69 mL, p<0.05).Conclusion: Prophylactic use of low-dose aprotinin immediately before cardiopulmonary bypass reduced the need for transfusion of blood & fresh frozen plasma (FFP) during the post operative period.

Prophylactic use of aprotinin has been shown to decrease postoperative blood loss and transfusion requirements in patients undergoing cardiac operations with extracorporeal circulation.
